目的探讨男性类风湿关节炎(rheumatoid arthriti,RA)患者骨量减低的相关情况及合并骨质疏松的危险因素。方法收集2013年8月至2019年5月在苏州大学附属常州市肿瘤医院诊治的男性类风湿关节炎患者38例,年龄(64. 39±9. 44)岁,对照组为40例健康正常人群,年龄(63. 53±8. 14)岁。通过双光能X线骨密度仪测定正位腰椎(L1~4)及左侧股骨近端(包括FN、Troch、Ward、Total)骨密度;运用电化学发光免疫分析法检测BAP、BGP、CTX;记录RA患者的ESR、CRP、关节肿胀及压痛数、RF以及CCP等情况。分析比较两组骨密度及骨生化指标的差异,并采用单因素分析和多因素Logistic回归分析调查类风湿关节炎患者合并骨质疏松的危险因素。结果两组间年龄、体质量指数(body mass index,BMI)差异无统计学意义(P0. 05)。类风湿关节炎组左侧股骨颈、整体髋骨BMD测量值较对照组减低(P0. 05);且类风湿关节炎组骨量减少、骨质疏松发生率较对照组显著增高(P0. 05);在骨生化指标方面,类风湿关节炎组CTX水平高于对照组(P0. 05)。单因素分析显示BMI、CRP、DAS28-ESR及CTX与男性类风湿关节炎患者继发骨质疏松相关(P0. 05);多因素Logistic回归分析显示,BMI 18. 5 kg/m2(OR=0. 014),DAS28-ESR5. 1(OR=21. 433)及CTX0. 7 ng/m L(OR=25. 875)是男性类风湿关节炎继发骨质疏松的独立危险因素(P0. 05)。结论男性类风湿关节炎患者同样易合并骨质疏松的发生,BMI、DAS28及CTX等危险因素应高度重视。 相似文献

目的探究卡马西平与左乙拉西坦治疗成人癫痫的效果及对认知功能、骨密度的影响。方法92例成人癫痫患者作为研究对象,随机分为左乙拉西坦组与卡马西平组,各46例。左乙拉西坦组采用左乙拉西坦治疗,卡马西平组采用卡马西平治疗。比较两组患者治疗前及治疗6个月后认知功能[语言智商(VIQ)、操作智商(PIQ)、总智商(FIQ)]评分、骨密度(腰椎、股骨大转子、股骨颈)变化情况。结果治疗6个月后,两组患者VIQ、PIQ、FIQ评分均较治疗前显著提升,且左乙拉西坦组患者VIQ评分(101.2±3.1)分、PIQ评分(108.1±2.3)分、FIQ评分(105.2±1.8)分均明显高于卡马西平组的(95.1±2.8)、(94.1±2.0)、(93.5±1.6)分,差异有统计学意义(P<0.05)。治疗6个月后,卡马西平组患者腰椎、股骨大转子、股骨颈骨密度均较治疗前显著下降,且明显低于卡马西平组,差异有统计学意义(P<0.05);左乙拉西坦组治疗6个月后腰椎、股骨大转子、股骨颈骨密度与治疗前比较差异无统计学意义(P>0.05)。结论左乙拉西坦与卡马西平治疗成人癫痫,左乙拉西坦更能明显提升患者认知功能,且对患者骨密度无影响。 相似文献

Nude mice have been extensively used to investigate the potency of tissue engineering strategies for bone repair. However, the contribution of pro‐inflammatory and proregenerative stimuli of the host for the process of new bone formation and integration remains poorly understood. In this study, ectopic bone formation was investigated in nude (Nu) versus wild‐type (WT) mice. Calcium phosphate (CaP) scaffolds (CopiOs [Zimmer] and Bio‐Oss [Geistlich]) were loaded with different concentrations of rhBMP6 (40, 120, and 240 ng/mm3 rhBMP6) and implanted subcutaneously in Nu (BALB/c and NMR1) and WT (BALB/c and c57BL/6) mice. CaP scaffolds loaded with rhBMP6 did not form bone in WT mice. However, in Nu mice, 40 ng/mm3 rhBMP6 was sufficient to generate relevant volumes of new bone at 6 weeks after implantation. Looking into potential underlying mechanisms, TNF‐α blocking antibodies were injected intraperitoneally but could not restore bone formation. Also, mouse periosteal cells (mPDCs) seeded in CopiOs loaded with rhBMP6 did not significantly improve the outcome. Abrogation of bone formation was associated with dense cellular infiltration, in particular with the presence of CD3+ T‐lymphocytes. To probe a correlation between calcium ions and impaired bone formation in WT mice, type 1 collagen gels were loaded with rhBMP6 and calcium chloride and injected subcutaneously. These gels generated new bone in WT mice despite the increased percentage of CD3+ cells at Day 3 after implantation as compared with control gels. Overall, this study illustrated the negative effect of the inflammatory host response on the bone‐forming capacity of rhBMP6 coated on bioceramic scaffolds. 相似文献

目的:探讨滤泡状甲状腺癌骨转移的临床特征和治疗方法。方法:回顾性分析1例以左侧锁骨病理性骨折及骨痛为首发症状的滤泡状甲状腺癌患者的临床影像学资料、病理学资料,随访治疗效果,并复习相关文献。结果:患者左侧甲状腺肿物考虑为滤泡状甲状腺癌。C5、T10椎体、左侧锁骨考虑为转移癌。行左侧锁骨部分切除术,术后病理提示:滤泡状甲状腺癌骨转移。结论:对以病理性骨折和骨痛为主要症状的滤泡状甲状腺癌,影像学与组织活检相结合有利于降低误诊率,治疗方案取决于对患者局部病灶和全身状况的全面评估。 相似文献

目的探讨经皮椎体后凸成形术(PKP)术中不同注入量高粘度骨水泥治疗骨质疏松性腰椎骨折(OLVF)的疗效及安全性。 方法前瞻性收集2016年9月至2018年9月本院OLVF患者150例,男84例,女66例,年龄(60±8)岁。依据随机数字表分为高量组、中量组、低量组,每组50例,高量组、中量组、低量组PKP术中高粘度骨水泥注入量分别为5.0~7.0 ml、3~4.9 ml、<3.0 ml,比较三组疗效及安全性。 结果150例患者获得满意随访,随访时间(19±7)个月。高量组、中量组、低量组手术时间、术中出血量比较,差异无统计学意义(P>0.05);高量组和中量组术后3、6个月椎体前缘高度[(27.3±3.1)mm、(26.0±2.7)mm和(26.9±3.0)mm、(25.7±2.8)mm]明显高于低量组[(23.8±2.8)mm、(21.3±2.5)mm],高量组和中量组术后3、6个月Cobb角及疼痛视觉模拟评分法(VAS)、Oswestry功能障碍指数问卷表(ODI)评分[(40.2±4.7)°、(41.5±4.8)°、(2.6±0.4)分、(1.6±0.3)分、(25.8±3.5)分、(26.9±3.5)分和(40.9±4.8)°、(42.1±4.8)°、(2.6±0.4)分、(1.6±0.3)分、(26.2±3.5)分、(27.2±3.7)分]明显低于低量组[(46.3±5.3)°、(47.8±5.6)°、(3.3±0.4)分、(2.3±0.4)分、(33.3±4.1)分、(34.3±4.2)分],差异有统计学意义(F=25.371、18.914、29.334、22.457、34.276、30.217、29.364、20.071,均P<0.001);高量组骨水泥渗漏率(28.00%)明显高于中量组和低量组(8.00%和4.00%),差异有统计学意义(χ2=10.241,P=0.005)。 结论PKP术中不同注入量高粘度骨水泥治疗OLVF的疗效及安全性存在一定的差异,其中注入中量(3~4.9 ml)高粘度骨水泥可获得良好的疗效及安全性,值得临床推广。 相似文献

Fracture non-union, prosthetic joint infection and fracture-related infections are complications frequently encountered by the trauma and orthopaedic surgeon. Moreover, they pose a significant socioeconomic and healthcare burden to the society. A clear understanding of the molecular biology surrounding fracture healing, as well as the pathophysiology of orthopaedic-related infections, is of paramount importance to their successful management. The first section of this article aims to explain the basic science critical to the understanding of DNA, RNA, gene and protein expression, and the molecular techniques most commonly deployed in the diagnostic and research fields of trauma and orthopaedics. BackgroundSmoking is a poor prognostic factor for healing after rotator cuff repair and is associated with inferior results. We hypothesized that smokers would have higher recurrent tear rates and more postoperative myotendinous junction (MTJ) retraction in healed repairs than nonsmokers three months postoperatively.MethodsRotator cuff repairs (RCRs) were retrospectively reviewed over a 2-year period. Patients underwent magnetic resonance imaging (MRI) within 6 months prior to surgery and again at 3 months postoperatively. Seventy-nine patients were included and stratified by smokers versus nonsmokers. Baseline patient demographics, tear characteristics, and surgical factors were collected. Preoperative and postoperative MRIs were assessed to quantify the MTJ position and to establish the recurrent tear rate.ResultsFor the total cohort (nonsmokers, n = 56; smokers, n = 23), significant differences in age, race, and traumatic onset of injury existed between groups. There were no significant differences in recurrent tear between smokers (26%) and nonsmokers (27%), but nonsmokers were more satisfied. For patients with healed RCRs (nonsmokers, n = 41; smokers, n = 17), there were significant differences in race. On univariate analysis, nonsmokers had a significantly more lateral MTJ postoperatively (P = 0.05). On multivariable regression analysis, medialized postoperative MTJ position in healed cuffs was driven only by greater preoperative rotator cuff retraction preoperatively. There were no significant differences in MTJ position based on smoking status for patients with healed RCRs.ConclusionSmoking does not appear to be an independent risk factor for postoperative MTJ retraction in healed RCRs, also known as failure in continuity. Preoperative tear size and retraction play the biggest roles in predicting postoperative MTJ position, regardless of smoking status. 目的探讨脱细胞真皮基质(ADM)膜联合骨修复材料在整复下颌骨缺损中的作用。方法在9只比格犬下颌骨两侧共建立36个下颌骨缺损模型,将模型随机分为A、B、C、D组,每组各9个。A组使用ADM膜+骨修复材料、B组使用Bio-Gide膜+骨修复材料、C组使用骨修复材料、D组为空白对照。分别于4、8、12 w各处死3只实验犬行大体及组织学观察,并评价螺旋CT值、Micro-CT数据和弹性模量数值。结果A、B两组新生骨情况明显优于C、D两组,且ADM膜和Bio-Gide膜具有相似作用。结论ADM膜具有良好的引导骨组织再生作用,其与Bio-Gide膜作用类似,ADM膜可在临床上推广使用。 相似文献
